TRAVEL DOCUMENT GENERAL INFORMATION
CIRCUMSTANCES FOR AVAILING A TRAVEL DOCUMENT:
- A Filipino national who has lost his/her Philippine passport while abroad; or,
- A Filipino national who have an urgent need to travel to the Philippines and cannot be issued OR do not have the opportunity to personally apply for a Philippine passport at the Philippine Embassy in Moscow.
The Travel document is valid for 30 days from the date of its issuance and CAN ONLY BE USED FOR ONE-WAY TRAVEL TO THE PHILIPPINES.
REQUIREMENTS FOR TRAVEL DOCUMENT:
- 3 pieces of 1.5x1.5 or 2x2 photo with white background
- old passport (expired)
- in case of lost passport, present the following:
- Report from the Police Station, with English translation, duly notarized by a Russian notary public and Apostilled by the Russian Ministry of Justice
- Affidavit of Lost Passport (form available at the Embassy)
- Copy of the lost passport (data page) and visa (if available)
The Consular Officer reserves the right to require additional documents pursuant to the Philippine Passport Law and the Foreign Service Act.
