GENERAL REQUIREMENTS

  1. Personal appearance of applicant (for biometric data capture)
    Note: there is no need to submit a passport photo as photo will be captured live, along with digital signature and fingerprints of applicant
  2. Duly accomplished application form -- download from here: (Regular-Adult-New) (Regular-Adult-Renewal) (Regular-Minor)
    Original passport to be renewed
  3. Photocopy of data page of passport being renewed
    including Amendment Page, if applicable
  4. Other Supporting Documents*

LIST OF SUPPORTING DOCUMENTS, ACCORDING TO THE FOLLOWING CONDITIONS:

A. FOR THOSE BORN IN RUSSIA:

Supporting Document/s:

  1. Birth Certificate authenticated by the Russian Ministry of Foreign Affairs

B. FOR THOSE WHO LOST OR MUTILATED THEIR PASSPORTS:

Supporting Document/s:

  1. Report from the Police Station, with English translation, duly notarized by a Russian notary public and Apostilled by the Russian Ministry of Justice
  2. Affidavit of Loss/Mutilation (form available at the Embassy)
  3. Copy of the lost passport (data page)
  4. Original Birth Certificate issued by the Philippine Statistics Authority (PSA, formerly NSO) –or– Report of Birth (if born abroad)*
  5. Original Marriage Certificate issued by PSA (formerly NSO) if married*
  6. Other documents

*Original PSA-issued documents will be returned to the applicant after verification

Note: 15-30 days clearing period for investigation and approval from the Department of Foreign Affairs (DFA) in the Philippines is required. A new passport may be issued to an applicant only after official endorsement/approval from DFA is received and after applicant has complied with all documentary requirements


C. FOR MARRIED WOMEN, WHO WANT TO USE SURNAME OF THEIR SPOUSE:

Supporting Document/s:

  1. Marriage Contract (MC) in Security Paper issued by the Philippine Statistics Authority (PSA, formerly the National Statistics Office or NSO) and Apostilled by the DFA (in DFA Aseana or its other Consular Offices)
    -or-
    Certified True Copy of MC issued by the Local Civil Registrar duly authenticated by PSA and Apostilled by the DFA

D. FOR MARRIED WOMEN WHO WOULD LIKE TO REVERT BACK TO THEIR MAIDEN NAME:

Supporting Document/s:

If widowed:

  1. Birth Certificate of the widow
  2. Death Certificate of the deceased husband, both issued by PSA and Apostilled by the DFA

If marriage is annulled:

  1. Certified True Copy and photocopy of the first page and the dispositive portion of the decree of annulment Apostilled by the DFA and PSA-issued
  2. Marriage Certificate with the annotation on the annulment decree, further Apostilled by the DFA

If divorced (in case that the marriage was contracted abroad then reported to PSA):

  1. Certified True Copy and photocopy of the first page and the dispositive portion of the judgment on divorce secured by foreign spouse, duly Apostilled by the competent authority of the country where the divorce was obtained
  2. PSA-issued Marriage Certificate with the annotation on the judgment on divorce, further Apostilled by the DFA         

E. FOR MINORS (BELOW 18 YEARS OLD):

Supporting Document/s:

  1. Photocopy of the data page of natural parent/legal guardian’s valid passport
  2. Birth Certificate issued by the Philippine Statistics Authority (PSA, formerly NSO) –or– Report of Birth (if born abroad)
  3. Marriage Certificate issued by PSA (formerly NSO) if the Minor Applicant is a legitimate child
  4. Personal appearance of either parent (if minor is of legitimate status)
  5. Personal appearance of mother (if minor is of illegitimate status)

If minor is NOT traveling with either parent:

  1. Affidavit of Support and Consent
  2. Passport and photocopy of the passport of the traveling companion of the minor
  3. Identification Card and photocopy thereof of the duly authorized person

F. FOR SEAFARERS:

Supporting Document/s:

  1. Certification from the corresponding shipping agency in Russia or from the ship master
  2. Philippine Seaman’s discharge book
  3. Landing permit issued by Russian immigration

TRAVEL DOCUMENT GENERAL INFORMATION

CIRCUMSTANCES FOR AVAILING A TRAVEL DOCUMENT:

  • A Filipino national who has lost his/her Philippine passport while abroad; or,
  • A Filipino national who has an urgent need to travel to the Philippines and cannot be issued a Philippine passport OR does not have the opportunity to personally* apply for a Philippine passport at the Philippine Embassy in Moscow.

*Applicable to Filipinos within the jurisdiction of the Philippine Embassy in Moscow, including Russia,
Belarus, Armenia and Kazakhstan.

The Travel document is valid for 30 days from the date of its issuance and CAN ONLY BE USED FOR ONE-WAY TRAVEL TO THE PHILIPPINES.
 
REQUIREMENTS FOR TRAVEL DOCUMENT:

  1. 3 pieces of 1.5x1.5 or 2x2 photo with white background
  2. old passport (expired)
  3. in case of lost passport, present the following:
  4. Report from the Police Station, with English translation, duly notarized by a Russian notary public and Apostilled by the Russian Ministry of Justice
  5. Affidavit of Lost Passport (form available at the Embassy)
  6. Copy of the lost passport (data page) and visa (if available)

The Consular Officer reserves the right to require additional documents pursuant to the Philippine Passport Law and the Foreign Service Act.
